Background: the biological activity of vitamin D depends on the activity of its receptor or VDR. On the other hand, the activity of this receptor is influenced by its state of methylation. The objective of this study was to verify if the BsmI polymorphism of the VDR gene influences its methylation profile in adolescents. Secondly, it was to verify if the status of some metabolic factors (oxidative stress, inflammation, lipid profile, and glycemia) in the serum, and gender-adjusted vitamin D levels are independent factors with an influence on the VDR methylation profile. Methods and results: the study included 198 adolescents of both sexes, aged 15-19 years, who underwent testing for VDR gene methylation polymorphisms, serum vitamin D levels, and metabolic, oxidative stress, and systemic inflammation markers. It was observed that the BB genotype was less methylated than the other groups (26.1 % versus 30.3 %, and 29.3 % for Bb and bb, respectively), although without statistical differences between them. The odds ratio indicated a protection of 13 % (partially methylated) for vitamin D status, while alpha glycols increased the risk ratio (of being partially methylated) by 3 %. MDA was protective at a 28 % chance of risk that adolescents with higher levels of lipid peroxidation would be hypomethylated. Conclusion: we conclude that the methylation profile of the VDR gene is not influenced by the different BsmI polymorphism genotypes, and that serum vitamin D and serum markers of oxidative stress and inflammation can modulate this profile. Antecedentes: la actividad biológica de la vitamina D depende de la actividad de su receptor, el VDR. Por otro lado, la actividad de este receptor está influenciada por su estado de metilación. El objetivo de este estudio es verificar si el polimorfismo BsmI del gen VDR influye en el perfil de metilación del mismo en los adolescentes. En segundo lugar, verificar si los factores metabólicos (estrés oxidativo, inflamación, perfil lipídico y glucemia) del suero y la vitamina D ajustada por sexo actúan independientemente de los polimorfismos sobre el perfil de metilación del VDR. Métodos y resultados: el estudio incluyó a 198 adolescentes de ambos sexos, de 15 a 19 años de edad, que se sometieron a análisis de polimorfismos de metilación del gen VDR, niveles de vitamina D, marcadores metabólicos, estrés oxidativo e inflamación sistémica. Se observó que el genotipo BB estaba menos metilado que los otros grupos (26,1 % contra 30,3 % y 29,3 % para Bb y bb respectivamente), aunque sin diferencias estadísticas entre ellos. El odds ratio indicó una protección del 13 % (parcialmente metilado) para el estado de la vitamina D, mientras que los alfa glicoles aumentaron el índice de riesgo (de estar parcialmente metilado) en un 3 %. La MDA fue protectora con un 28 % de probabilidad de riesgo de que los adolescentes con niveles más altos de peroxidación lipídica fueran hipometilados. Conclusión: concluimos que el perfil de metilación del gen VDR no está influenciado por los diferentes genotipos del polimorfismo BsmI y que la vitamina D y los marcadores de estrés oxidativo e inflamación en el suero pueden modular este perfil. Methylenetetrahydrofolate reductase (MTHFR) C677T polymorphism associated with body fat accumulation could possibly trigger an inflammatory process by elevating homocysteine levels and increasing cytokine production, causing several diseases. This study aimed to evaluate the effects of food intervention, and not folate supplements, on the levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-1ß (IL-1ß) in overweight and obese women with the MTHFR C677T polymorphism. A randomized, double-blind eight-week clinical trial of 48 overweight and obese women was conducted. Participants were randomly assigned into two groups. They received 300 g of vegetables daily for eight weeks containing different doses of folate: 95 µg/day for Group 1 and 191 µg/day for Group 2. MTHFR C677T polymorphism genotyping was assessed by digestion with HinfI enzyme and on 12% polyacrylamide gels. Anthropometric measurements, 24-h dietary recall, and biochemical analysis (blood folic acid, vitamin B12, homocysteine (Hcy), TNF-α, IL-1ß, and IL-6) were determined at the beginning and end of the study. Group 2 had a significant increase in folate intake (p < 0.001) and plasma folic acid (p < 0.05) for individuals with the cytosine-cytosine (CC), cytosine-thymine (CT), and thymine-thymine (TT) genotypes. However, only individuals with the TT genotype presented reduced levels of Hcy, TNF-α, IL-6, and IL-1ß (p < 0.001). Group 1 showed significant differences in folate consumption (p < 0.001) and folic acid levels (p < 0.05) for individuals with the CT and TT genotypes. Food intervention with folate from vegetables increased folic acid levels and reduced interleukins, TNF-α, and Hcy levels, mainly for individuals with the TT genotype.

Objective: to verify the association of serum concentrations of 25-hydroxyvitamin D and glycemic levels with the genetic variants rs1544410 and rs2228570 of the VDR gene in adolescents from the Northeast region of Brazil. Materials and methods: a cross-sectional epidemiological study with 208 adolescents from public schools in the city of João Pessoa (Paraíba, Brazil) between 15 and 19 years of age. Blood samples were collected for DNA extraction and analysis of polymorphisms rs1544410 and rs2228570, as well as biochemical analyses (25-hydroxyvitamin D, parathyroid hormone, calcium and glycemia). Results: the mean age was 17.7 (± 1.14) years. Half of adolescents had sufficient serum levels of 25-hydroxyvitamin D and the other half had insufficient/deficient vitamin. The most frequent genotypic distribution was bb and Ff and of lesser frequency BB and ff. There was a significant relationship between the genotypes of rs1544410 and glycemia values (p = 0.049) in the relationships between the genotypes BBxbb (p = 0.012) and Bbxbb (p = 0.037); (p = 0.036, OR = 2.15, 95 % CI = 1.05-4.41), and in the BB+Bb group analysis when compared to the bb (p = 0.025, OR = 1.89, 95 % CI = 1.08-3.29) presented higher risk of glycemia above the median. On the other hand, when Bb+bb was analyzed in relation to BB, adolescents had a greater chance of blood glucose below the median (p = 0.025, OR = 0.66, CI = 0.47-0.95). Conclusion: this study showed a significant relation of glycemia with the distribution of rs1544410 polymorphism genotypes


Objetivo: verificar la asociación de las concentraciones séricas de 25-hidroxivitamina D y los niveles de glucemia con las variantes genéticas rs1544410 y rs2228570 del gen VDR en adolescentes de la región noreste de Brasil. Materiales y métodos: se realizó un estudio epidemiológico transversal con 208 adolescentes de escuelas públicas en la ciudad de João Pessoa (Paraíba, Brasil) de entre 15 y 19 años de edad. Se recogieron muestras de sangre para la extracción de ADN y el análisis de los polimorfismos rs1544410 y rs2228570, así como para análisis bioquímicos (25-hidroxivitamina D, hormona paratiroidea, calcio y glucemia). Resultados: la edad media fue de 17,7 (± 1,14) años. La mitad de los adolescentes tenía niveles séricos suficientes de 25-hidroxivitamina D y la otra mitad, vitamina insuficiente/deficiente. La distribución genotípica más frecuente fue bb y Ff y la de menor frecuencia, BB y ff. Hubo una relación significativa entre los genotipos de rs1544410 y los valores de glucemia (p = 0,049) en las relaciones entre los genotipos BBxbb (p = 0,012) y Bbxbb (p = 0,037); (p = 0,036, OR = 2,15, IC 95 % = 1,05-4.41), y el análisis del grupo BB + Bb en comparación con el bb (p = 0,025, OR = 1,89, IC 95 % = 1,08-3,29) mostró un mayor el riesgo de glucemia, por encima de la mediana. BACKGROUND: Defects in DNA methylation have been shown to be associated with metabolic diseases such as obesity, dyslipidemia, and hypercholesterolemia. To analyze the methylation profile of the ADRB3 gene and correlate it with lipid profile, lipid intake, and oxidative stress based on malondialdehyde (MDA) and total antioxidant capacity (TAC), homocysteine and folic acid levels, nutritional status, lifestyle, and socioeconomic variables in an adult population. A cross-sectional epidemiological study representative of the East and West regions of the municipality of João Pessoa, Paraíba state, Brazil, enrolled 265 adults of both genders. Demographic, lifestyle, and socioeconomic questionnaires and a 24-h recall questionnaire were applied by trained interviewers' home. Nutritional and biochemical evaluation (DNA methylation, lipid profile, MDA, TAC, homocysteine and folic acid levels) was performed. RESULTS: DNA hypermethylation of the ADRB3 gene, analyzed in leukocytes, was present in 50% of subjects and was associated with a higher risk of being overweight (OR 3.28; p = 0.008) or obese (OR 3.06; p = 0.017), a higher waist-hip ratio in males (OR 1.17; p = 0.000), greater intake of trans fats (OR 1.94; p = 0.032), higher LDL (OR 2.64; p = 0.003) and triglycerides (OR 1.81; p = 0.031), and higher folic acid levels (OR 1.85; p = 0.022). CONCLUSIONS: These results suggest that epigenetic changes in the ADRB3 gene locus may explain the development of obesity and non-communicable diseases associated with trans-fat intake, altered lipid profile, and elevated folic acid. Because of its persistence, DNA methylation may have an impact in adults, in association with the development of non-communicable diseases. This study is the first population-based study of the ADRB3 gene, and the data further support evaluation of ADRB3 DNA methylation as an effective biomarker.

The C677T polymorphism of the methylenetetrahydrofolate reductase gene (MTHFR) is related to folate metabolism and can alter the levels of biochemical markers.Objective: Investigate the influence of the MTHFR C677T polymorphism on the effects of a dietary folate intervention on oxidative stress in women with overweight or obesity.Methods: Forty-eight adult women with overweight or obesity were subjected to a 24-hour dietary recall, anthropometric measurements, biochemical analysis, and genotyping of the MTHFR C677T polymorphism. They were allocated by convenience sampling to 2 groups, which received 300 g of folate-rich vegetables containing 191 µg/d (Group 1) (n = 24) or 95 µg/d (Group 2) (n = 24) of folate for 8 weeks.Results: The dietary intervention increased the serum folic acid levels in the 2 analyzed groups. The intervention with 191 µg/d of folate led to relevant results in terms of homocysteine levels (p = 0.0005) and total antioxidant capacity (p = 0.0261); the effect was larger among carriers of the TT genotype.Conclusions: The study demonstrated the beneficial effect of folate intake in terms of a TAC elevation for the CC and TT genotypes of the MTHFR C677T polymorphism, an increase in folic acid levels for all genotypes, and a reduction in the Hcy levels for the TT genotype in response to an intervention consisting of an intake of 191 µg/d of folate supplied by vegetables.

ABSTRACT Objective The present study aimed at investigating the association between hematological profile and serum 25-hydroxyvitamin D (25[OH]D) levels and Fokl polymorphism of the vitamin D receptor gene in individuals with Cystic Fibrosis. Methods A cross-sectional study that involved 18 men and women aged 0-25 years with Cystic Fibrosis. Socio-demographic information and the factors associated with sun exposure were obtained. Weight, height, and arm circumference were also measured. Blood sample was collected for the analysis of biochemical parameters (25[OH]D, parathyroid hormone, and calcium levels and blood count) and for the validation of the presence of FokI polymorphism in the vitamin D receptor gene. Results Among the participants, 33.33% (n=6) had vitamin D deficiency (19.60±6.180 ng/mL), and 27.8% (n=5) presented with anemia and low weight for age. In terms of genotype, 5.6% (n=1) presented with the FF genotype, 72.3% (n=13) had the Ff genotype, and 22.2% (n=4) had the ff genotype. Serum 25(OH)D levels were associated with hemoglobin (p=0.008) and hematocrit (p=0.019) levels and leukocyte count (p=0.0114). No association was observed between 25(OH)D levels and the genotypes (FF, Ff, and ff) (p=0.2451). In addition, an association was observed between FokI polymorphism and the total leukocyte count (p=0.01). Conclusion An association was observed between serum 25(OH)D levels and hemoglobin and hematocrit levels and leukocyte count in individuals with Cystic Fibrosis. Moreover, FokI polymorphism was associated with total leukocyte count.


RESUMO Objetivo Esta pesquisa teve por objetivo investigar a associação do perfil hematológico com os valores séricos de 25-hidroxivitamina D e polimorfismo FokI do gene receptor da vitamina D em indivíduos com fibrose cística, acompanhados em um centro de referência do Nordeste Brasileiro. Métodos Trata-se de estudo transversal, realizado com 18 indivíduos com fibrose cística, de ambos os sexos, com idade entre zero e 25 anos. Foram coletadas informações sociodemográficas e investigados fatores associados à exposição solar. Também foram aferidas medidas antropométricas de peso, estatura e circunferência do braço. O sangue foi coletado para análise dos parâmetros bioquímicos (25-hidroxivitamina D, paratormônio, cálcio e hemograma) e verificação da presença do polimorfismo FokI do gene receptor da vitamina D. Resultados Da amostra total, 33,33% (n=6) apresentaram insuficiência/deficiência de vitamina D (19.60 ±6.180 ng/ml), e 27,8% (n=5) acusaram anemia e baixo peso para a idade. No tocante aos genótipos, 5,6% (n=1) apresentaram genótipo FF, 72,3% (n=13) apresentaram genótipo Ff e 22,2% (n=4) apresentaram genótipo ff. Houve associação entre os valores séricos de 25-hidroxivitamina D e os de hemoglobina (p=0.008), hematócrito (p=0.019) e leucócitos (p=0.0114). Não houve associação entre os valores de 25-hidroxivitamina D e os genótipos (FF, Ff e ff) (p=0.2451). Além disso, houve associação entre o polimorfismo FokI e a contagem total de leucócitos (p=0.01). Conclusão O presente estudo encontrou associação entre os valores séricos de 25-hidroxivitamina D e os de hemoglobina, hematócrito e leucócitos nos indivíduos analisados. Além disso, encontrou-se associação do polimorfismo FokI com a contagem total de leucócitos.

Objetivo: O objetivo deste estudo foi investigar a associação do retinol sérico, proteína C-reativa ultra-sensível (PCR-us) e a ingestão de fibras alimentares em uma população de idosos hipertensos. Material e Métodos: Trata-se de um estudo transversal de base populacional com 170 idosos com idade entre 60 e 90 anos, de ambos os sexos, de uma cidade do Nordeste do Brasil. Para as análises bioquímicas, as concentrações de retinol sérico e PCR-us foram analisadas e um questionário quantitativo de frequência de alimentar auto-administrado foi coletado. Foram utilizados como critérios de inclusão: idosos de 60 a 90 anos que residiam na cidade de João Pessoa e hipertensão auto-relatada. Os critérios de exclusão foram indivíduos com transtornos neuropsiquiátricos e indivíduos que relataram suplementação de vitamina A, carotenóides ou suplementos de fibras. Os dados foram analisados com o teste de Wilcoxon, teste exato de Fisher e análise de regressão logística. Todas as análises estatísticas foram realizadas com o Software R Development Core Team. Resultados: Não foram observadas relações significativas entre hipertensão arterial e níveis séricos de retinol (p = 0,4325), níveis de PCR (p = 0,4104) e consumo de fibra alimentar (p = 0,0935). Com base na regressão logística, cada aumento de 1 unidade no índice de massa corporal (IMC) aumenta a probabilidade de hipertensão em 0,009545%. As outras variáveis não contribuíram para o modelo de regressão final. Conclusão: Considerando a alta prevalência de indivíduos hipertensos com valores adequados de retinol, não houve associação entre retinol sérico, níveis de PCR e ingestão de fibra. A maioria dos participantes apresentou níveis normais de PCR-us, que podem ter sido influenciados pela atividade antioxidante do retinol e pela ingestão inadequada de fibras. (AU)


Objective: To investigate the association between serum retinol, highlsensitivityC- reactive protein (hs-CRP) and dietary fiber intake in a population of hypertensive elderlies. Material and Methods: This was a cross-sectional population-based study with170 elderly people aged 60-90 years, of both sexes, from a city in northeastern Brazil. Biochemical analyses were performed, in which serum retinol and hs- CRP concentrations were analyzed, and a self-administered quantitative food frequency questionnaire was applied. Inclusion criteria were: 60-90 years old, living in the city of Joao Pessoa, and selfreported hypertension. Exclusion criteria were: individuals with neuropsychiatric disorders, and individuals who reported supplementation of vitamin A, carotenoids or fiber supplements. The data were analyzed using Wilcoxon test, Fisher's exact test and logistic regression analysis. All statistical analyses were performed with the R Development Core Team Software. Results: No significant relationships were observed between hypertension and serum retinol levels (p = 0.4325), hs-CRP levels (p = 0.4104) and dietary fiber intake (p = 0.0935). Based on logistic regression, each 1 unit increase in body mass index (BMI) increases the probability of hypertension by 0.009545%. The other variables did not contribute to the final regression model. Conclusion: Considering the high prevalence of hypertensive individuals with adequate retinolvalues, there was no association between serum retinol, CRP levels and fiber intake. The majority of participants had normal hs-CRP levels, which may have been influenced by the antioxidant activity of retinol and inadequate fiber intake. (AU)

Introdução: Nesta pesquisa, examinou-se a ocorrência de síndrome metabólica, diabetes mellitus e obesidade e as suas relações com o consumo habitual de macro e micronutrientes e grupos de alimentos, em mulheres com antecedentes de diabetes mellitus gestacional (DMG). Método: O estudo envolveu 110 pacientes atendidas no ambulatório de diabetes gestacional de um hospital universitário localizado em uma cidade do Nordeste do Brasil, das quais 49 completaram todas as etapas da investigação. No período pós-parto entre 6 meses e 4 anos, as pacientes recrutadas para uma primeira consulta responderam questionários acerca dos seus antecedentes clínicos, hábitos alimentares e do nível de atividade física e submeteram-se à aferição do peso e altura, realização de exame de bioimpedância e exames laboratoriais, a serem apresentados na segunda consulta. Resultados: A prevalência de síndrome metabólica foi elevada, totalizando 49% das pacientes; a de diabetes mellitus foi de 16%; e de disglicemias de 65%, porcentagens próximas àquelas encontradas em estudos realizados durante cinco anos após a gestação ou por períodos mais longos. Do total das pacientes, 45% estavam com sobrepeso e 28% obesas. Após análise estatística, observou-se relação entre a prevalência de síndrome metabólica e menor ingestão de proteínas (p=0,05), quando os macro e micronutrientes foram ajustados pelo peso. Conclusão: Não houve relação entre o consumo alimentar habitual e a ocorrência de diabetes mellitus e obesidade e a frequência de síndrome metabólica nas pacientes com diagnóstico prévio de DMG foi elevada, sendo sua ocorrência associada à menor ingestão de proteínas.

Comparar os valores de nutrientes encontrados nosrótulos de alimentos enriquecidos com ácido fólico e ferrocomercializados em João Pessoa com os valores recomendadospela RDC n. 344 da ANVISA de 2002, e com os encontrados naTabela Brasileira de Composição de Alimentos e no softwareDietwin. Material e Métodos: A amostra foi composta pelolevantamento de dados sobre a composição de ácido fólico eferro em 833 alimentos, contendo farinha de milho e farinhade trigo nos dois supermercados que comercializam produtosem nível nacional, localizados no município de João Pessoaonde foram realizas 14 visitas. Todas as análises estatísticasforam realizadas com o auxílio do software Core RDevelopment Team, para verificar as associações, utilizou-seo Teste Qui-quadrado com a concentração dos nutrientes,originados das três fontes de dados. Resultados: Dos 833alimentos analisados, 111(13%) apresentavam valores nosrótulos, 535(64%) tinha a informação de enriquecimento nosingredientes e 187(23%) não apresentavam informação deenriquecimento nos ingredientes. Ao comparar-se os valoresde ácido fólico e ferro, constatou-se que os valores descritosnos rótulos quanto ao ácido fólico eram semelhantes aosvalores encontrados no software Dietwin, e com relação aoferro, os valores encontrados nos rótulos eram superiores aosencontrados no software Dietwin como também em relação aTabela Brasileira de Composição dos Alimentos. Conclusão:No presente estudo observou-se que a fortificação com ácidofólico foi ligeiramente superior ao recomendado com umaumento médio de 10,7%, já a fortificação com ferro supriauma quantidade duas vezes maior, em média, que a obrigatóriaem 26% dos rótulos analisados, com base na legislação vigente...


To compare the nutrient content claimed in folicacid- and iron-enriched food labels commercialized in the cityof João Pessoa, PB, Brazil, with that recommended by theBrazilian Health Surveillance Agency (ANVISA) Decree no.344/2002, and by the Brazilian Table of Food Compositionand Dietwin software. Material and Methods: The sample wascomposed by data about folic acid and iron content in 833food items containing corn and wheat flour. The survey wascompleted after 14 visits in two supermarkets located in JoãoPessoa, which sell national products. All statistical analyseswere performed using the Core R Development Team software.In order to check associations between the nutrient contents inthe three data sources, the Qui-square test was used. Results:Of the 833 items analyzed, 111(13%) presented values in thelabels, 535 (64%) disclosed the information on ingredientenrichment and 187(23%) lacked this sort of information. Wefound that the values described in the labels about folic acidwere similar to those found in Dietwin software, whereas thevalues claimed for iron were greater than the ones found inDietwin software and also in the Brazilian Table of FoodComposition. Conclusion: Folic acid enrichment was slightlyhigher than the recommended with a mean increase of 10.7%,whereas iron enrichment had a two-fold higher amount onaverage than the required one in 26% of the labels analyzedbased on current legislation...

OBJECTIVE: In Brazil, demographic, socioeconomic and epidemiological changes over time have led to a transition in nutritional standards, resulting in a gradual reduction of malnutrition and an increased prevalence of overweight and obese individuals, similar to the situation in developed countries in previous decades. This study assessed the body mass index (BMI) and the prevalence of an overweight status and obesity, adjusted for various factors, in a population in northeastern Brazil including all age groups. METHODS: This is a cross-sectional population-based epidemiological study using single sampling procedure composed of levels. Given the heterogeneity of the variable "income" and the relationship between income, prevalence of diseases and nutrition, a stratified sampling on blocks in the first level was used. In this, city districts were classified by income into 10 strata, according to information obtained from IBGE. A systematic sampling was applied on randomly selected blocks in order to choose the residences that would be part of the sample (second level), including 1165 participants from all age groups. RESULTS AND DISCUSSION: The prevalence of an overweight status or obesity was adjusted for demographic, socioeconomic and lifestyle variables. When the Chi-square test was applied, a relationship was observed between the prevalence of an overweight status or obesity and the age group, gender, educational level and income of the participants. Regarding lifestyle parameters, only smoking was associated with the prevalence of an overweight status or obesity, in both adults and in the total sample. The results for the following groups were significant (p < 0.05): the age group from 20 to 59 years, when the individual presented an educational level greater than or equal to high school; and the age group ≥ 60 years, when the individual was female. It is noteworthy that educational level and being female were significant in adjusting for the total population as major factors influencing an increased BMI, followed by the variables physical activity and family income. CONCLUSIONS: The adjusted results justify the adoption of intervention and prevention policies to combat these clinical conditions for the study population as a whole, particularly directed toward adults with higher education level as well as elderly females.

Pomacea lineata is a fresh water shellfish that is utilized as food, medicinal portions, or licking remedies. The objective of this study was to evaluate the nutritional and microbiological aspects of P. lineata and spawning. The samples were collected in the Jaguaribe River (Paraíba, Brazil) during the rainy and dry seasons. The biochemical composition, mineral composition, total mesophilic bacteria (TMB), total coliforms (TC), fecal coliforms (FC), Salmonella sp., Staphylococcus aureus, and Vibrio presence were determined. Each 100 g of P. lineata presented 3.1 g of ashes; 9.4 g of proteins; 2.7 g of lipids; 0.6 g of carbohydrates; 1800.0 mg of calcium; 78.0 mg of phosphorus, and 58.4 mg of iron. Each 100 g of spawning presented 10.4 g of ashes; 4.5 g of proteins; 2.8 g of lipids; 1.3 g of carbohydrates; 3633.0 mg of calcium; 39.4 mg of phosphorus; and 4.6 mg of iron. Each 100 g of P. lineata provided 65.7 kcal and spawning 48.3 kcal. In the dry season P. lineata and spawning showed no TMB and TC, FC, S. aureus, Salmonella, or Vibrio. In the rainy season P. lineata and spawning presented increase in TMB and TC, FC, S. aureus, and Salmonella. The presence of Vibrio was not detected. P. lineata presented proteins, low lipid, and carbohydrate content, energy values close to those of lean meats, high contents of calcium and iron, and presented low contamination level with pathogenic bacteria. Since P. lineata substances may be useful in the treatment of respiratory and gastrointestinal diseases without presenting toxic or cytotoxic effects already described in the literature, it may be considered as a potential functional food.

OBJECTIVES: To evaluate the effect of Bach flower Rescue Remedy on the control of risk factors for cardiovascular disease in rats. DESIGN: A randomized longitudinal experimental study. METHODS: Eighteen Wistar rats were randomly divided into three groups of six animals each and orogastrically dosed with either 200 µl of water (group A, control), or 100 µl of water and 100 µl of Bach flower remedy (group B), or 200 µl of Bach flower remedy (group C) every 2 days, for 20 days. All animals were fed standard rat chow and water ad libitum. MAIN OUTCOME MEASURES: Urine volume, body weight, feces weight, and food intake were measured every 2 days. On day 20, tests of glycemia, hyperuricemia, triglycerides, high-density lipoprotein (HDL) cholesterol, and total cholesterol were performed, and the anatomy and histopathology of the heart, liver and kidneys were evaluated. Data were analyzed using Tukey's test at a significance level of 5%. RESULTS: No significant differences were found in food intake, feces weight, urine volume and uric acid levels between groups. Group C had a significantly lower body weight gain than group A and lower glycemia compared with groups A and B. Groups B and C had significantly higher HDL-cholesterol and lower triglycerides than controls. Animals had mild hepatic steatosis, but no cardiac or renal damage was observed in the three groups. CONCLUSION: Bach flower Rescue Remedy was effective in controlling glycemia, triglycerides, and HDL-cholesterol and may serve as a strategy for reducing risk factors for cardiovascular disease in rats. This study provides some preliminary "proof of concept" data that Bach Rescue Remedy may exert some biological effects.

BACKGROUND: Low calcium:phosphorus ratios (Ca:P ratio) in habitual diet have been observed worldwide, and it has been shown to be harmful to the bone health of the population. However, no study associating this ratio with obesity was found. Thus, considering that the intake of calcium and phosphorus will generate a ratio between them, which may be associated with obesity, this research seeks at evaluating the relation between obesity and the Ca:P ratio in the habitual diet of adults. METHODS: Cross-sectional population-based epidemiological study with stratified and systematic sampling. The sample was composed of 506 adults, aged between 18 and 60 years, of both genders. Information on socioeconomic and demographic conditions was obtained through questionnaires completed during home visits, where anthropometric and dietary evaluations were also conducted. RESULTS: In the habitual diet consumed by the study subjects, a Ca:P ratio above the median of 0.57 reduced the risk of central obesity based on waist-to-height ratio (WHtR) (OR: 0.61; 95% CI: 0.41 - 0.92). Habitual dietary intake of calcium (OR: 0.65; 95% CI: 0.43 - 0.97) and dairy products (OR: 0.56; 95% CI: 0.37 - 0.84) above the median value (485.4 mg and 0.9 servings, respectively) was found to be a protective factor related to central obesity based on WHtR. CONCLUSIONS: Values above the median for the Ca:P ratio found in the habitual diet were negatively associated with central obesity based on WHtR. In addition, calcium and dairy consumption were negatively associated with central obesity based on WHtR. Therefore, higher Ca:P ratios contributed to a lower prevalence of central obesity.

OBJECTIVE: To develop and apply a new instrument to evaluate prenatal care based on the guidelines of the Brazilian Humanization Program for Prenatal Care and Birth, including quantitative and qualitative elements distributed according to the evaluation triad of structure, process, and outcomes. METHODS: An analytic, descriptive, quantitative cross-sectional study was carried out with 238 women receiving care in 44 primary care services in the city of João Pessoa, Northeast Brazil, between November 2010 and December 2011. Physicians and nurses involved in prenatal care were also interviewed. The instrument developed by the investigators contains 23 questions relating to structure, process and outcomes of pre-natal care. Based on the information collected, pre-natal care was classified according to an IPR/Pre-Natal index (Índice IPR/Pré-Natal, where I stands for infrastructure, P for work process and R for results). A value of 1 is attributed to each question if it complies with the criteria established for quality, or 2 if it does not comply. The percent of adequate answers in relation to the 23 total questions is used to classify prenatal care as: upper adequate (100% adequate answers); adequate (>75%); intermediate (51 to 74%); and inadequate (<50%). The classification categories for the Pre-Natal/IPR index were compared to those of the Kessner and Adequacy of Prenatal Care Utilization (APNCU) indices. RESULTS: Questions relating to the work process contributed significantly to the classification of prenatal care as intermediate according to the IPR/Pre-Natal. IPR/Pre-Natal classification categories were consistent to detect prematurity, insufficient weight at birth and absence of exclusive breastfeeding. CONCLUSIONS: The IPR/Pre-Natal index effectively incorporated quantitative and qualitative elements for the assessment of pre-natal care.

OBJETIVO: Desenvolver e aplicar um novo instrumento para avaliar o pré-natal com base nas diretrizes do Programa de Humanização do Pré-Natal e do Nascimento do Ministério da Saúde brasileiro, inserindo elementos quanti-qualitativos distribuídos segundo a tríade avaliativa de infra-estrutura, processo de trabalho e resultados. MÉTODOS: Este estudo transversal, de natureza analítico-descritiva e abordagem quantitativa, foi desenvolvido com 238 usuárias em 44 serviços da atenção primária à saúde do Município de João Pessoa, Nordeste do Brasil, de novembro de 2010 a dezembro de 2011. Também foram entrevistados médicos e enfermeiros envolvidos no cuidado pré-natal. O instrumento elaborado pelos pesquisadores continha 23 questões relativas a infra-estrutura, processo de trabalho e resultados da assistência pré-natal. A partir das informações coletadas, o pré-natal foi classificado pelo que denominamos de Índice IPR/Pré-Natal. Para cada questão atribui-se o valor 1 para adequado, quando em conformidade com os critérios estabelecidos, e 2 para inadequado. Com base na porcentagem representada pela soma do número de questões adequadas dos três componentes em relação ao total de 23 questões, o cuidado pré-natal foi classificado em: adequado superior (100% de questões adequadas); adequado (>75%); intermediário (51 a 74%); e inadequado (<50%). As categorias de classificação do IPR/Pré-Natal foram comparadas com as dos índices de Kessner e Adequacy of Prenatal Care Utilization (APNCU). RESULTADOS: As questões referentes ao processo de trabalho foram as que contribuíram com significância estatística para que o pré-natal fosse classificado em maior proporção na categoria intermediário pelo Índice IPR/Pré-Natal. As categorias de classificação do Índice IPR/ Pré-Natal mostraram-se consistentes para detectar as variáveis prematuridade, peso insuficiente ao nascer e não realização do aleitamento materno exclusivo. CONCLUSÕES: O Índice IPR/Pré-Natal foi eficaz na incorporação de elementos quanti-qualitativos para classificação do cuidado pré-natal.

Caracterizar a assistência pré-natal em Unidades básicas de Saúde envolvendo profissionais e usuárias do município de João Pessoa-PB. MÉTODOS: estudo do tipo transversal e analítico-descritivo. A fim de caracterizar o pré-natal, um instrumento específico foi elaborado com base nas diretrizes do Programa de Humanização do Pré-natal e Nascimento reunindo questões distribuídas e analisadas segundo a tríade avaliativa: estrutura, processo e resultados. RESULTADOS: observou-se boa infra-estrutura em maior parte dos serviços; quanto ao processo, identificou-se baixa adesão aos requisitos mínimos do Programa de Humanização do Pré-natal e Nascimento (39,9 por cento), além de baixo percentual de serviços com atividades de educação em saúde (45,5 por cento) e avaliação interna (47,7 por cento). Para os resultados da assistência, altos percentuais de gestantes com intercorrências clínicas (60,9 por cento), inadequada situação nutricional (56,3 por cento sobrepeso e obesidade) e baixa prevalência de aleitamento materno exclusivo (58,05 ± 34,19 dias) foram observados. CONCLUSÕES: aspectos de processo referentes à adesão ao pré-natal, presença de serviços com atividades de educação em saúde e indicadores de avaliação interna além de questões de resultados envolvendo intercorrências clínicas, inadequada situação nutricional na gestação e baixa prevalência de aleitamento materno exclusivo são barreiras à qualidade das ações de pré-natal no âmbito da atenção primária à saúde...


To characterize prenatal care at Basic Health Units regarding health workers and users in the municipality of João Pessoa, in the Brazilian State of Paraíba. METHODS: a cross-sectional, analytical-descriptive study was carried out. In order to characterize prenatal care, a specific tool was developed based on the guidelines of the Humanization of Prenatal Care and Birth Program, gathering together questions under the headings, structure, process and results. RESULTS: satisfactory infrastructure was found in the case of most services. In the case of the process, the study identified a low level of adherence to the minimal requirements of the Humanization of Prenatal Care and Childbirth Program (39.92 percent), and a low proportion of services including health education (45.5 percent) and internal evaluation (47.7 percent). In terms of the results of care, a high percentage of pregnant women with clinical irregularities (60.9 percent), inadequate nutritional status (56.3 percent overweight or obese) and a low level of exclusive maternal breastfeeding (58.05 ± 34.19 days) were observed. CONCLUSIONS: aspects of the process relating to prenatal care, the existence of services incorporating health education and internal evaluation indicators and the issue of results involving clinical irregularities, inadequate nutritional status during pregnancy and low levels of exclusive maternal breastfeeding stand in the way of achieving high quality in primary prenatal healthcare...

INTRODUCTION: Data already demonstrated the increased prevalence of obesity in various segments of the population. In this context, bariatric surgery is accepted nowadays as the most effective tool in the treatment and control of morbid obesity. Several studies have evaluated the nutritional status after bariatric surgery, especially mixed by detecting a reduction in food intake of protein, vitamins and minerals. However, other studies have investigated the presence of nutritional deficiencies prior to surgery, which may be aggravated by the surgical procedure, resulting in serious postoperative complications. AIM: To analyze the literature in relationship to micronutrient deficiencies in obese patients before bariatric surgery. METHOD: Was carried out a systematic review in several electronic databases, such as PubMed/ Medline, Scielo, Lilacs and Bireme. The following keywords were used alone or in combination: bariatric surgery, obesity, preoperative, gastric bypass, vitamin deficiencies, deficiencies and mineral nutrient absorption. Were included 40 review and original articles published between 2005 to 2012. RESULTS: There were consensus on the combination of preoperative nutritional deficiencies, restrictions and malabsorption, possibly induced by bariatric surgery that can lead patients to experience significant nutritional deficits during the late postoperative period, especially of micronutrients, resulting in serious complications. CONCLUSIONS: The high occurrence of micronutrient deficiencies preoperatively detected in morbidly obese candidates for bariatric surgery, plus a malabsorptive procedure, may involve worse prognosis during the late postoperative period. Preoperative evaluation of nutritional parameters and food intake is recommended in conjunction with surgical interventions.

INTRODUÇÃO: Achados epidemiológicos têm demonstrado o aumento da prevalência de obesidade em diversos segmentos da população mundial. Neste contexto, a cirurgia bariátrica é aceita, atualmente, como a ferramenta mais eficaz no tratamento e controle da obesidade mórbida. Vários estudos vêm avaliando o estado nutricional após operações bariátricas, principalmente as mistas, detectando redução no consumo alimentar de proteínas, vitaminas e minerais. No entanto, outros investigam a presença de deficiências nutricionais antes da realização da operação, que podem ser agravadas com o procedimento cirúrgico, resultando em complicações pós-operatórias graves. OBJETIVO: Analisar a literatura para as deficiências de micronutrientes em pacientes obesos antes da cirurgia bariátrica. MÉTODO: Realizou-se revisão sistemática em bases eletrônicos, a saber: PubMed/Medline, Scielo, Lilacs e Bireme. As seguintes palavras-chave foram utilizadas individualmente ou em associação: bariatric surgery, obesity, preoperative, gastric bypass, vitamin deficiencies, mineral deficiencies and absorption nutrient. Foram incluídos 40 artigos de revisão e originais, publicados entre 2005 a 2012. RESULTADOS: Encontrou-se como consenso o fato de que a combinação de deficiências nutricionais no pré-operatório, as restrições e má absorção induzidas pela cirurgia bariátrica podem levar a importante déficits nutricionais durante o período pós-operatório tardio, principalmente de micronutrientes, resultando em complicações graves. CONCLUSÃO: A alta ocorrência de deficiência de micronutrientes no pré-operatório detectada em obesos mórbidos candidatos à cirurgia bariátrica, somado a um processo disabsortivo, pode envolver pior prognóstico durante o período pós-operatório tardio. Avaliação pré-operatória dos parâmetros nutricionais e da ingestão de alimentos é recomendado em conjunto com as intervenções.


INTRODUCTION: Data already demonstrated the increased prevalence of obesity in various segments of the population. In this context, bariatric surgery is accepted nowadays as the most effective tool in the treatment and control of morbid obesity. Several studies have evaluated the nutritional status after bariatric surgery, especially mixed by detecting a reduction in food intake of protein, vitamins and minerals. However, other studies have investigated the presence of nutritional deficiencies prior to surgery, which may be aggravated by the surgical procedure, resulting in serious postoperative complications. AIM: To analyze the literature in relationship to micronutrient deficiencies in obese patients before bariatric surgery. METHOD: Was carried out a systematic review in several electronic databases, such as PubMed/ Medline, Scielo, Lilacs and Bireme. The following keywords were used alone or in combination: bariatric surgery, obesity, preoperative, gastric bypass, vitamin deficiencies, deficiencies and mineral nutrient absorption. Were included 40 review and original articles published between 2005 to 2012. RESULTS: There were consensus on the combination of preoperative nutritional deficiencies, restrictions and malabsorption, possibly induced by bariatric surgery that can lead patients to experience significant nutritional deficits during the late postoperative period, especially of micronutrients, resulting in serious complications. CONCLUSIONS: The high occurrence of micronutrient deficiencies preoperatively detected in morbidly obese candidates for bariatric surgery, plus a malabsorptive procedure, may involve worse prognosis during the late postoperative period. Preoperative evaluation of nutritional parameters and food intake is recommended in conjunction with surgical interventions.
